Pothole and manhole cover

The manhole cover is very loose and makes a very loud noise every time a car goes over it which is constantly. The road is surface is breaking up around the pothole as well and with a constant stream of cars and lorry’s it’s getting worse and dangerous now. Can this be looked at urgently and repaired as soon as possible
