Worn/invisible road markings on J29 M25/A127 roundabout & poor signage, particularly on eastern side of the roundabout. Lane 1 is supposed to be for Works Traffic Only but impossible to tell anymore. This results in people wanting A127 West sitting in Lane 2 (for M25 South) instead of Lane 3 (Other Routes) then preventing people from merging after the traffic lights from Lane 3 into Lane 1 for the A127 West exit from the roundabout. See near-collisions here almost every day with people being in the wrong lane.
