Lane markings and signage at this location to clarify which lanes should be used for a right turn

I am writing to report a safety concern regarding the M55 westbound off‑slip at Junction 4 (Preston New Road/A583).
The slip road initially has two lanes, and the left lane splits again after leaving the motorway. There appears to be no clear lane markings or arrows, which has caused confusion among drivers on the roundabout. I have witnessed numerous near misses, especially for vehicles intending to take the right turn at the exit as some use the middle lane as well as the right lane.
Could you please review the lane markings and signage at this location to clarify which lanes should be used for a right turn, and consider any improvements to reduce the risk of collisions?
Thank you for your attention to this safety concern. I would appreciate confirmation that this issue is being reviewed.
