Awful ridges/failure of road surface

The whole of the A5 southbound carriage way between Old Stratford and Abbey Hill roundabout is in an awful failing condition. As well as various potholes or cracked central sections, four large separate perpendicular ridges across the whole cariageway at different points because of bridge subsidence is causing suspensions to be hammered. It's really bad. Please send out a survey crew. The whole carriageway needs scraping off and resurfacing properly. Please urgently investigate.
