Badly sunken inspection cover Lane 2 of slip road from M25 westbound to M23 southbound

Hi, on this slip road, at the end of the sweeping bend onto the M23 but just before the 200yds signage there's a sunken inspection cover in lane 2.

I travel this way weekly and it's getting steadily worse. Now gives quite a jolt, albeit that there's currently no hard edges to it as the asphalt has sunk with it. At a guess it's now about 50 to 80mm deep.
