Road surface breaking up

Sent to National Highways less than a minute later. FixMyStreet ref: 6188183.

Yesterday a chunk of road pulled up as the car in front drove over it and it flicked up and damaged my windscreen badly enough to need replacing! Please address this safety issue urgently, now you are informed if it happens again I will be looking for the price of the next windscreen to be covered by highways England!
