Lengthy potholes in the middle lane

Sent to National Highways less than a minute later. FixMyStreet ref: 6155313.

Long length potholes around 2 to 4 metres and around 1 to 1.5 inches depth in the midde lane mostly, with drivers verring out of the way of them into the left and right lane, causing near collisions, happens every evening on my way home. Someone is going to have a serious accident, as I have reported this before and still nothing done in last two months.
