Potholes on slip road leaving Leeds

Sent to National Highways less than a minute later. FixMyStreet ref: 5984346.

Significant/deep potholes covering the entry on to the slip road to J4 (M621) exiting Leeds. This covers the left hand lane of the slip road which has very deep/large potholes, which is exposing the layer beneath on the road. Cars are having to brake/slow down to mitigate the impact of running over these. Potential to damage cars. Issue has been present for several months (google maps displays June 2023 on the same area of road showing potholes which are much worse now). Please investigate/repair the road.
