Loose manhole cover - very noisy for residents

A manhole cover is loose, every time a vehicle goes over it it moves causing a significant noise. This goes on all day and night and for local residents (especially with children or pets) causes a big disturbance. This has been going on now for at least 5 weeks if not longer and is getting worse.
