Badly sunken manhole cover on very busy main road - looks like it could collapse - sunken by approx 4 inches from road surface

I was cycling on the pavement and noticed the manhole cover which was severely sunken (this was yesterday). I also drive on this road and the volume of traffic is very heavy, including many HGVs. I think this depressed manhole has recently occurred.
