Road surface breaking up / dipping

Sent to National Highways less than a minute later. FixMyStreet ref: 4322682.

The East bound carriageway between jct 6 and jct 5 is breaking up in several places. The surface also dips badly (sink hole?) in one 5 meter stretch.
