Large part of road surface crumbling on entry ramp, very dangerous

Large part of the entry ramp (spans whole width) - cars pulling out prematurely onto A34 cause drivers to brake suddenly. It has been progressively getting worse for a month and now is so large cars bounce along through it unless avoiding. It is at least 10m long and 3m wide
