Road marking confusing

Witnessed an accident this morning due to confusing road markings. When coming off the M25 there are 4 lanes on the exit slip road, 1 for A12(W) 2 for A12 (E) and 1 for the A1023 coming up to the set of traffic lights. This then turns in to 3 lanes 1 for the M25(N), 1 for A12(E) and 1 for A1023/M25S. As you come round the corner past the entrance slip road to the M25N the 3 lanes then turn to 2 for A12E and 1 for A1023. The issue is that people are in the left hand lane for the M25N coming off the M25 expecting it to go round to the A12, should this lane not say M25N/A12(E) to avoid confusion?

This remains an issue, and there are multiple reports of this problem and collisions/near collisions as a consequence. The fix is very simple to mark this lane as M25N/A12(E) to avoid confusion, and it's difficult to see why this has not been done.
